- To avoid errors, information should not be filled directly in the proposal form. First fill the information in customer information sheet, get it checked from an experienced person and then fill up original proposal form.
- Fill complete information. Write clearly. Use only black ink or black ball pen.
- Following documents should be attached with the form. Company will not accept the form if the required documents are found short.
- Photo Identification Proof (Anyone)-Driving License/Passport / Pan Card / Voter ID Card.
- Age Proof (Anyone) - Mark Sheet of X or XII wherein mentioned date of Birth/Pan Card/Driving License/Passport.
- Address Proof (Anyone) - Passport / Driving License / Ration Card.
- 2 Passport Size colour photographs. (Write your name and proposal form no. at the back of the photographs)
Note :- If you submit driving License or passport proof then there is no need of any other document.
- Policy Premium will be payable only by way of DD in favour of "BAJAJ ALLIANZ LIFE INSURANCE COMPANY LIMITED" (Payable at Bhilwara, Raj.)
- Premium will be accepted annually only.
- All the attached documents should be signed by the proposer.
- Pin code is essential with address of proposer along with telephone no. so that policy can be sent to the correct address.
- There should be no cutting and overwriting in the form and if done should signed by the proposer.
- Fill "know your customer form" and sign at x mark.
- Sign at all the places where there is x mark in the form.
